Independent investigation of the Boeing Starliner mission (CFT) in 2026 revealed critical flaws in NASA management, by classifying the incident 2024 year as an accident of the highest level (Type A), comparable to the Challenger and Columbia space shuttle disasters, which highlights dramatic failures, which left two astronauts stranded in space for nine months. The report points to prioritizing app security over security, ineffective oversight of Boeing and serious errors in management decision-making.
The investigators provided 61 official recommendation to that effect, what NASA and Boeing can do, to prevent similar mistakes in the future, and NASA Administrator Jared Isaacman said: "We must admit our mistakes and ensure, so that they never happen again".
The report was published at the time, as NASA prepares its Artemis II mission, as part of which a crew of four astronauts for the first time for over 50 years will fly around the moon, and Boeing is the prime contractor for the Space Launch System rocket, which it will provide.
